1.  Fill out and sign registration forms. (Dayley Dance Academy Class Registration Forms)

2.  Make check payable to Dayley Dance Academy for first month’s tuition to ensure placement in desired class.

3.  Mail to the Dayley Dance Academy at 15206 NE 48th St. Vancouver, WA 98682

* Remember that space is limited to the first 6-8 students that send in their registration.
